Catholic Health's Good Shepherd Hospice provides palliative care services and hospice services to support patients and families throughout the entire course of a serious illness.

Our dedicated team offers care wherever needed, including in a patient's or family member's home, assisted living facilities, skilled nursing facilities, and hospitals. We also provide short-term inpatient care at our Hospice Centers in Port Jefferson on the campus of St. Charles Hospital and in Rockville Centre within Mercy Hospital.
